This form is a short screener. We use it to decide whether (a) we should invite you to schedule a consultation/case review, (b) we should refer you elsewhere, or (c) we need more information first.

Deadlines matter in aviation. Please answer date questions as precisely as you can. If you don’t know an exact date, give your best estimate and tell us what you used to estimate it (email date, pay period, calendar entry, etc.).

Be specific. Short, concrete facts help us evaluate your options faster than general statements.

Have key documents handy if possible: any FAA letters/notices (LOI, NOV, subpoena, order, proposed action, certificate action), medical correspondence (AME deferral, FAA request letters, denial letters), NTSB correspondence, and transaction documents (draft purchase agreement, title/registration, logbooks summaries). If you don’t have everything, submit what you do have—the most recent FAA/NTSB document and key dates are the most helpful.
